THE ITALIAN PASSPORT IS A TRAVEL DOCUMENT AND A FORM OF ID THAT IS ISSUED ONLY FOR ITALIAN CITIZENS. THOSE INDIVIDUALS WHO THINK MAY BE ENTITLED TO ITALIAN CITIZENSHIP MUST CONTACT THE CITIZENSHIP DEPARTMENT OF THE LOS ANGELES CONSULATE GENERAL AND OBTAIN ITALIAN CITIZENSHIP BEFORE THEY CAN APPLY FOR AN ITALIAN PASSPORT.

FROM MONDAY, MARCH 22ND 2010 THE ITALIAN HONORARY VICE CONSULATE OF PHOENIX WILL NOT HANDLE ANY LONGER PASSPORTS FILINGS. PASSPORTS WILL ONLY BE DONE BY THE ITALIAN CONSULATE GENERAL OF LOS ANGELES. ITALIAN CITIZENS IN NEED OF A PASSPORT NEED TO MAKE AN APPOINTMENT WITH THE ITALIAN CONSULATE OF LOS ANGELES, THROUGH THEIR WEB SITE, AND GO IN PERSON TO LOS ANGELES.

Exceptions to the New Regulations are:

1) Passports for minors less than 12 years of age

2) The Consent Form to the issue of the passport for the spouse

That will continue to be made by the Italian Consulate of Arizona.

The Consulate General of Italy in Los Angeles will implement the second phase of the electronic passport project and, starting on March 22nd 2010, will issue only the new type of biometric passport which will include, in addition to the picture, the holder’s fingerprints. These electronic passports will be valid for ten years.

Therefore from the above date, fellow citizens requesting a passport will necessarily have to come in person to Consulate General to provide their fingerprints. This service cannot be rendered by Honorary Consulates or Consular Correspondents.

To reduce inconveniences and waiting periods, the General Consulate of Italy in Los Angeles provides an online appointment scheduling system that can be accessed through the web page indicated below. The appointments system has greatly contributed to the elimination of queues and waiting periods, thus obtaining remarkable approvals.

The Consulate General of Italy will continue to pursue solutions to facilitate, within the framework of the new laws, access to the services provided by the Passport office.

Italian passports are issued for 10 years, After 10 years it is going to be necessary to request the issue of a new passport. A new passport can be issued as:

1. First passport.

2. Renewal of an expired passport.

3. Replacement of a lost or stolen passport.

The Italian Citizens with USA residence, in case of lost or stolen passport or in addition to the documentation requested will need to obtain also the documents listed under point 4 of the paragraph: Stolen or Lost Passport at the end of this web page.

To obtain your new passport, you will need to set-up an appointment with the Arizona Honorary Consul. The office of the Consul is open by appointment only. Italian USA residents must also be registered with the AIRE at the Los Angeles Consulate General; see AIRE under Services on this Web Site. You will need to prepare the following documentation. All forms will have to be signed in front of the Consul.

1. Expired passport, if any and photocopy of the picture page.

2. Arizona Driving License and photocopy.

3. Issuance form for adults or Issuance form for minors duly filled out; both parents and the kid(s) must be present at the appointment.

4. 3 passport size photos 1.5" X 1.5" on white background taken no later than six months before (only photos of very good quality - "homemade" photos are not accepted).

5. Alien Registration Card and photocopy or long term valid visa and photocopy.

In case of an American citizen:

U.S. Passport (if US citizen by birth)

Naturalization Certificate (if US citizen by naturalization)

6. Spouse consent form when there are minor children (less than 18 yrs old). Parents must present proof of identity and certified copy of the children’s birth certificate with the name of both parents. Parents/Spouses not E.U. citizens (e.g. husband/wife American only) must sign the form in front of a Consular Officer or Honorary Consular Officer.

The signature can be also notarized through a Public Notary. In this case the form must be accompanied by an "Apostille" issued by the Secretary of State.

7. Money Order for the exact amounts required, only Money Order. The fee requested will be the fee of the day of issuance of the passport. Therefore, in case of change of the consular fees (every 3 months), the Consulate will ask for the difference in case of a fee increase (compared to the amount paid at moment of the request) or it will reimburse the difference in case of a fee reduction.

8. Form to the Consent to the mailing of the passport.

9. If applicant wishes that the passport be returned by mail, as opposed to personally picking it up, For security reasons, this office requires that Applicant enclose one of the following PRE-PAID ENVELOPES: "US POST OFFICE EXPRESS MAIL" or “FEDEX USA AIRBILL” (price varies according to distance). Mailing label sections "TO" and "FROM" must BOTH contain applicant's Name and Address, NOT the Consulate address. This again is for security reasons. This Envelope will be included with the Passport filing and mailed with all other documents to the Italian Consulate General of Los Angeles. The person requesting the passport will do its own mailing. This Consulate will not retain any documents or do the mailing. The Italian Consulate General of Los Angeles is the only authority for the issuance of the new passport.



Stolen or Lost Passport - ISSUANCE OF A TRAVEL DOCUMENT

If you are in an emergency situation (i.e. you are a tourist that should leave immediately and your passport has been lost or stolen) and there is no time to wait for a "nulla-osta" authorization from the questura or other consulate to issue a new passport, the Consulate General of Los Angeles can issue a Travel Document valid only to go back to Italy. In this case, the following documents must be presented in person, at the Italian Consulate General of Los Angeles (Only the Italian Consulate General of Los Angeles can issue the Travel Document, not the Arizona Consulate):

1.Issuance form for adults duly filled and signed.

2. Valid ID with photo.

3. 2 passport size photos on white background taken no later than six months before (only photos of very good quality are accepted).

4.Loss or Theft Declaration and a Report from the local police that your Italian passport was lost or was stolen.

5. Confirmed Air ticket or E-Ticket.
